Believe I found part of the answer -- the cookie_secure was .warmglass instead of .warmglass.com The three other cookie items are all .warmglass.com
Fixing that allowed me to log in. It didn't tell me why ACP Add User extension not working or why can't stay logged in from session to session.
At any rate, force_server_vars = 1, script_path = /phpBB3, server_name = warmglass.com (no leading period), server_port = 443, and server_protocol = https://
